2012-2013 ENROLLMENT POLICY AND PROCEDURES
Complete Enrollment Contract - Please read your enrollment contract carefully. Once received by St. Luke’s this contract is legally binding. Use one contract per student.
- Complete student information, parents’ names, home address, and home phone number.
- St. Luke’s parishioners must circle the correct response on the contract concerning good standing with the church. We will assume the answer is no if nothing is chosen and the discount for enrollment fee will not be given.
- Mark Choice of Payment for tuition.
- Both parents must sign and date the contract.
- Return both copies of the contract. A copy of the fully executed contract will be returned to you after April 1, 2012.
Attach Enrollment Fee - Fee is due when contract is returned. Please note: current students will be charged an additional amount if contract and fee are not returned by February 1, 2012.
Attach Bank Draft Authorization and $5.00 fee (if applicable) - Monthly tuition payment (option 3) The Bank Draft Authorization form must be returned with the contract. Monthly payments must be made by automatic bank draft which will begin on April 5, 2011 and continue on the 5th of each month until March 5, 2013. Automatic Bank Draft Authorization form
Return to the Admission Office - Contract and fees are due in the Admission Office no later than 4:00 p.m., Monday, February 1, 2012. Any questions regarding the contract or enrollment process should be directed to in the admission office at 225-927-8601.
